Milnacipran is suited for development to treat FMS. Increasing evidence suggests that the pain in FMS is centrally mediated, and the modulation of more than one neurotransmittr system is likely to be required for optimal efficacy in treating the pain associated with this condition. Milnacipran, the first of a new class of agents known as NSRI's, or Nonepineprhine Serotonin Reuptake Inhibitors, acts on the two key neurotransmitters in the human body which are involved with the central modulation of chronic pain.

The annual costs associated with Fibromyalgia Syndrome (FMS) are estimated to total billions of dollars. Over 70% of the U.S. population will at some point suffer from an FSS. Somatic complaints account for 50-60% of office visits in the primary care setting. FMS is estimated to affect between 2 to 4% of the population, and is the second most commonly diagnosed rheumatological disorder after osteoarthritis. The symptoms of FMS are chronic and widespread pain and stiffness throughout the body accompanied by severe fatigue, poor sleep and headache. There is also a high incidence of associated neurologic, allergic, cardiac, pulmonary, gastrointestinal, genitourinary, and affective symptoms among sufferers. Patients with FMS have at least comparable disability, more pain, and lower quality of life than patients with rheumatoid arthritis or osteoarthritis.
